Elevating Healthcare Equity: CHC's CalAIM Journey with Aztute

We are thrilled to announce that our esteemed partner, the California Health Collaborative (CHC), took center stage at this year's #OPENMINDS conference to shine a spotlight on how Aztute's innovative solution is driving transformative care across California's ambitious CalAIM program. 

Covering 14 counties and continuing to expand, CHC stands as one of the largest and most forward-thinking Community-Based Organization (CBO) providers in the state. Beyond its exceptional support for the groundbreaking CalAIM initiative, CHC also champions Perinatal Programs, Breast & Cervical Health, Tobacco Prevention, Nutrition, Cancer Registry Services, Senior Services, Healthcare Support Services, Youth Development, and Mental Health programs. 

CalAIM (California Advancing and Innovating Medi-Cal), led by the California Department of Health Care Services (DHCS), represents a game-changing endeavor to transform the state's Medicaid program, Medi-Cal. With its multi-year commitment, CalAIM boasts a wide array of objectives, from enhancing healthcare access and eradicating health disparities to streamlining administrative processes and promoting value-based care models. 

By ensuring that every Californian has equitable access to top-notch healthcare and fostering improved overall health outcomes, CalAIM is poised to make a lasting impact on the lives of individuals facing complex medical, behavioral health, and social challenges across the Golden State.
